Application for Donations
Checklist

Your organization is eligible if:

• You are a 501(c)3 or public eqivalent
• You provide direct services to clients on-site
• Your location has secure areas for art which are used regularly by clients
• You do not have access to funds which could otherwise be used for the purchase of art

Eligible nonprofits must complete a simple application process.

Application for artwork donations

• Submit a complete application (See application check list for requirments)

Application form (pdf) - Acrobat Reader required

Donee agreement (pdf) - Acrobat Reader required

• The Art Connection staff will arrange to make a site visit to your location to review places for artwork, discuss your application and review next steps

• We will then make a final determination on your application.

Once qualified you will move to the next step: choosing artwork

Application check list

Please include the following:

• Completed Application form (pdf) - Acrobat Reader required
• Signed Donee agreement (pdf) - Acrobat Reader required (please send one & keep one for your records)
• Copy of 501(c)(3)
• Most recent Annual Report (or 990 or Audit/Review statement)
• Brochure or other information
• List of board members

We cannot consider incomplete applications for placements.
